Forms & Policy

Required forms:

Before your first session, I require your health history as well as the permission/referral of your physician for those of you who are pregnant.

Please read the important policies below.

We understand that unanticipated events occasionally happen in everyone’s life. However, we desire to be respectful and fair to all clients. Please be aware that there are many current clients as well as new clients on the waitlist; each person is hopeful of receiving a notice of cancellation. We, therefore, ask that you please acknowledge the following office policies:

  • 36-Hour Cancellation Policy: A minimum 36-hour notice is required when canceling an appointment. This advance notice on your part allows us to make your appointment time available to others. Regardless of whether you find yourself experiencing COVID exposure, a fever, or a personal setback, you will be charged full price for any cancellation that is less than 36 hours before your appointment time. FULL PAYMENT for any late cancellation must be received before attending your next appointment.
  • No-Show PolicyAnyone who forgoes their appointment without notice, for any reason, will be considered a “no-show” and charged full price for the missed appointment. FULL PAYMENT for missed sessions must be received before attending your next appointment.
  • Late Arrival PolicyIf you arrive late, your session may be shortened to maintain the day’s schedule and respect all subsequent appointment times. Upon arrival, the therapist will determine if enough time remains to begin your session. Regardless of the treatment’s duration, Full Payment is required. Out of respect and consideration for your therapist and other clients, please make every effort to be on time to receive the session’s full benefit.
